Nurse's Notes - Required Immunizations

Parents are requested to cooperate with the school nurse in completing the legal requirements for immunizations (below). Exemptions from immunization allowed by the law will be honored. However, NYS now requires that a Medical Immunization Exemption Statement, Medical Immunization Exemption Statement, completed by the child's physician, is submitted annually. Parents are reminded that the law requires that students not meeting all requirements may be excluded from school.

NOTE:  NYS Law now requires all children entering or attending seventh and twelfth grades on or after September 1, 2016, must receive an adequate dose or doses of vaccine against meningococcal disease as recommended by the Advisory Committee on Immunization Practices. PLEASE CONTACT YOUR CHILD'S PHYSICIAN FOR MORE INFORMATION.

NYS Immunization Requirements (call your child's physician with any questions)

Grades K-5

  • Diphtheria & Tetanus toxoid-containing vaccine and Pertussis vaccine (DtaP/DTP/Tdap/Td): 4-5 doses if the 4th dose was received at 4 years or older or 3 doses if 7 years or older and the series was started at 1 year or older
  • Polio vaccine (IPV/OPV): 4 doses or 3 doses if the 3rd dose was received at 4 years or older
  • Measles, Mumps, and Rubella vaccine (MMR):  2 doses
  • Hepatitis B vaccine: 3 doses or 2 doses of adult hepatitis B vaccine (Recombivax) for children who received the doses at least 4 months apart between the ages of 11-15 years
  • Varicella (chickenpox) vaccine: 2 doses

Grades 6-11

  • Diphtheria & Tetanus toxoid-containing vaccine and Pertussis vaccine (DtaP/DTP/Tdap/Td):  3 doses
  • Tetanus and Diphtheria toxoid-containing vaccine and Pertussis vaccine adolescent booster (Tdap):  1 dose
  • Polio vaccine (IPV/OPV): 4 doses or 3 doses if the 3rd dose was received at 4 years or older
  • Measles, Mumps, and Rubella vaccine (MMR):  2 doses
  • Hepatitis B vaccine: 3 doses or 2 doses of adult hepatitis B vaccine (Recombivax) for children who received the doses at least 4 months apart between the ages of 11-15 years
  • Varicella (chickenpox) vaccine: 2 doses
  • Meningococcal Conjugate vaccine (MenACWY): Grades 7-11--1 dose

Grade 12
