Nurse's Notes-Required Immunizations

Parents are requested to cooperate with the school nurse in completing the legal requirements for immunizations (below). Exemptions from immunization allowed by the law will be honored. However, NYS now requires that a Medical Immunization Exemption Statement,   Medical Immunization Exemption Statement, completed by the child's physician, is submitted annually. Parents are reminded that the law requires that students not meeting all requirements may be excluded from school.

NOTE:  NYS Law now requires all children entering or attending seventh and twelfth grades on or after September 1, 2016 must receive an adequate dose or doses of vaccine against meningococcal disease as recommended by the Advisory Committee on Immunization Practices. PLEASE CONTACT YOUR CHILD'S PHYSICIAN FOR MORE INFORMATION.
